Application For Transcript

Requesting Procedure

1. Download the Transcript Request Form in PDF format.
2. Fill the form with accurate details.
3. Submit the filled form to the dean's Office either as a printed document or via email (arfos@esn.ac.lk).
4. Ensure that you provide the bank receipt along with the form (If submitting by email, please attach a scanned copy of the receipt).

Note: Transcripts send only to the Universities, Ministries, Embassies, or other Recognized Institutes acceptable to Authority.
